Trametinib je hrazen v kombinaci s dabrafenibem u dospělých pacientů maligním melanomem s mutací V600 genu BRAF (potvrzenou před zahájením léčby pomocí validovaného testu), se stavem výkonnosti 0 – 1 dle ECOG:
1) k léčbě neresekovatelného nebo metastatického melanomu u pacientů, kteří nebyli v minulosti léčeni systémovou terapií pro inoperabilní pokročilé či metastatické onemocnění. Terapie je hrazena do progrese onemocnění.
Terapie je hrazena pouze za předpokladu kumulativního splnění následujících podmínek:
a) pacient nevykazuje přítomnost symptomatických mozkových metastáz a primárního okulárního melanomu;
b) pacientova předpokládaná doba života je delší než 4 měsíce;
c) pacient má laboratorní hodnoty krevního obrazu a biochemické hodnoty funkce ledvin umožňující léčbu.
2) v adjuvantní léčbě pacientů ve stádiu III nebo pacientů s rozsahem nemoci odpovídajícím stádiu III, po kompletní chirurgické resekci.
Pacient nevykazuje přítomnost primárního okulárního melanomu.
Terapie je hrazena do rekurence onemocnění či nezvládnutelné toxicity, maximálně však po dobu 12 months.
