a) jako udržovací léčba tam, kde není možno dosáhnout a trvale udržet astma pod kontrolou podáváním inhalationho kortikosteroidu v nízké dávce a při dobré spolupráci pacienta.
b) jako udržovací a úlevová léčba v rámci strategie (S)MART (single inhaler/maitenance and reliever therapy) na 3. – 5. stupni tíže astmatu
c) jako úlevová léčba v rámci strategie „AIR-only“ (antiinflamatory reliever only) na stupni 1 a 2 tíže astmatu.
2) Pneumolog předepisuje léčbu inhalation fixní kombinací budesonid/formoterol u dospělých pacientů s chronickou obstrukční plicní nemocí (CHOPN) s přítomností nejméně jednoho z následujících klinických fenotypů: frekventní exacerbátor (2 a více exacerbace za posledních 12 měsíců) a/nebo nemocný s překryvem CHOPN a astmatu (ACOS/ACO). Podmínkou úhrady v této indikaci je dobrá spolupráce pacienta nekuřáka či kuřáka spolupracujícího na zanechání kouření, definovaného jako kuřáka, který je plně adherentní, tj. dodržuje léčebný/pohybový režim, užívá správně všechny předepsané léky a minimálně jednou u něho byla provedena a zdokumentována cílená intervence v rámci léčby závislosti na tabáku.
