Azacitidin je hrazen z prostředků veřejného zdravotního pojištění v první linii léčby dospělých pacientů se stavem výkonnosti dle ECOG 0-2, kteří nejsou způsobilí k transplantaci hematopoetických kmenových buněk a jejichž klinický stav umožňuje léčbu:
– myelodysplastických syndromů (MDS) intermediárního rizika 2. stupně a vysokého rizika podle Mezinárodního prognostického skórovacího systému (IPSS);
– chronické myelomonocytové leukémie (CMML) s 10-29 % blastů v kostní dřeni bez myeloproliferativního onemocnění;
– akutní myeloidní leukémie (AML) s 20-30 % blastů a dysplazií ve více buněčných liniích, podle klasifikace Světové zdravotnické organizace (WHO).
Terapie se ukončí kdykoliv z důvodu neakceptovatelné toxicity nebo progrese onemocnění (nárůst počtu blastů v kostní dřeni o více nebo rovno 25 %). Po 6 podaných cyklech se terapie ukončí, pokud není dosaženo alespoň hematologického zlepšení (pokles počtu blastů v kostní dřeni o více nebo rovno 25 %). Pokud po 6 podaných cyklech došlo alespoň k hematologickému zlepšení (pokles počtu blastů v kostní dřeni o více nebo rovno 25 %), pacientům je hrazena terapie do neakceptovatelné toxicity nebo progrese onemocnění (nárůst počtu blastů v kostní dřeni o více nebo rovno 25 %).
