1) u pacientů s metastatickým kastračně rezistentním karcinomem prostaty se stavem výkonnosti 0-2 dle ECOG dříve léčených docetaxelem, u kterých došlo k progresi onemocnění. Pacienti mohou být předléčeni jednou až dvěma liniemi chemoterapie (docetaxelem). Léčba je hrazena do progrese onemocnění (rentgenologické progrese nebo výskytu nepříznivé skeletální příhody)
2) u pacientů s metastatickým kastračně rezistentním karcinomem prostaty se stavem výkonnosti dle ECOG 0-1, kteří jsou asymptomatičtí nebo mírně symptomatičtí po selhání androgenní deprivační léčby a u nichž dosud nebyla chemoterapie klinicky indikována, a u nichž bolest způsobená maligním onemocněním dosahuje na škále BPI (Brief Pain Inventory) pouze hodnot 0-1 (tj. bez symptomů bolesti). Terapie abirateronem je hrazena do progrese onemocnění.
3) v kombinaci s androgen deprivační terapií (ADT) u pacientů s nově diagnostikovaným metastatickým hormonálně senzitivním karcinomem prostaty (mHSPC) s vysokým rizikem (podmínka splnění 2 ze 3 následujících kritérií: Gleason skóre větší nebo rovno 8, přítomnost 3 a více kostních metastáz, přítomnost viscerálních metastáz), kteří ADT dosud nebyli léčeni nebo u nichž byla léčba zahájena během posledních 3 měsíců, o stavu výkonnosti ECOG PS 0-1. Léčba je hrazena do progrese (klinické nebo radiologické) nebo do výskytu nezvládnutelné toxicity.
