1) žen s postmenopauzální osteoporózou a mužů s osteoporózou, u obou pohlaví po selhání předchozí minimálně 2 roky trvající a spolehlivě dokumentované antiresorpční léčby u nemocných s T-skóre bederní páteře nebo proximálního femuru či krčku femuru (při nemožnosti měření v oblasti bederní páteře) menší nebo rovno -2,5 SD a zároveň s více než jednou zlomeninou obratlových těl v anamnéze, není-li dále uvedeno jinak.
2) žen a mužů léčených glukokortikoidy v denní dávce odpovídající 5 mg nebo více prednisonu po dobu nejméně 6 měsíců, pokud mají T-skóre v oblasti bederní páteře nebo proximálního femuru či krčku femuru (při nemožnosti měření v oblasti bederní páteře) menší nebo rovno -2,5 SD.
3) postmenopauzálních žen s těžkou osteoporózou s vysokým rizikem opakovaných zlomenin definovanou prokázanou ztrátou kostní hmoty celotělovým denzitometrem (T-skóre menší nebo rovno -2,5 SD na standardních měřených místech (bederní páteř, proximální femur event. předloktí) a proděláním osteoporotické (křehké) zlomeniny.
