Venetoklax je hrazen:
1) v kombinačním režimu s obinutuzumabem k léčbě dospělých pacientů s dříve neléčenou chronickou lymfocytární leukémií (CLL) a s komorbiditami, v důsledku kterých nemohou podstoupit léčbu na bázi fludarabinu v plné dávce. Jedná se o pacienty o stavu výkonnosti 0-1 dle ECOG. Přípravek je hrazen do vyčerpání 12 cyklů terapie nebo do progrese onemocnění či výskytu nepřijatelné toxicity, dle toho, co nastane dříve.
2) v kombinačním režimu s rituximabem u dospělých pacientů s relabující nebo refrakterní chronickou lymfocytární leukémií (CLL) o stavu výkonnosti 0-1 dle ECOG, kteří splňují alespoň jedno z následujících kritérií:
(a) jsou refrakterní na poslední léčbu;
(b) došlo u nich k relapsu do 24 měsíců po ukončení předcházející léčby;
(c) došlo u nich k relapsu a nejsou vhodní na chemo-imunoterapii;
(d) je u nich prokázaná mutace TP53 nebo del17p.
Přípravek je hrazen do progrese onemocnění (tj. i po vyčerpání 6 předepsaných cyklů rituximabu) nebo do projevů nepřijatelné toxicity, nejdéle po dobu 24 měsíců od 1. dne 1. cyklu s podáním rituximabu, dle toho, co nastane dříve.
