1) Alergolog/klinický imunolog a pneumolog předepisuje léčbu inhalation fixní kombinací flutikason furoát/vilanterol u nemocných s diagnózou perzistující asthma bronchiale tam, kde není možno dosáhnout a trvale udržet asthma pod kontrolou podáváním inhalationho kortikosteroidu v nízké dávce a při dobré spolupráci pacienta.
2) Pneumolog předepisuje léčbu inhalation fixní kombinací flutikason-furoát/vilanterol u symptomatických dospělých pacientů s chronickou obstrukční plicní nemocí (CHOPN) s postbronchodilatační hodnotou FEV1 méně než 50 % náležité hodnoty a současně s přítomností nejméně jednoho z následujících klinických fenotypů: frekventní exacerbátor (2 a více exacerbace za posledních 12 měsíců) a/nebo nemocný s překryvem CHOPN a astmatu (ACOS/ACO). Podmínkou úhrady v této indikaci je dobrá spolupráce pacienta nekuřáka či kuřáka spolupracujícího na zanechání kouření, definovaného jako kuřáka, který je plně adherentní, tj. dodržuje léčebný/pohybový režim, užívá správně všechny předepsané léky a minimálně jednou u něho byla provedena a zdokumentována cílená intervence v rámci léčby závislosti na tabáku.
