1a) u dospělých imatinibem předléčených pacientů v chronické fázi, akcelerované fázi nebo blastické fázi chronické myeloidní leukemie s pozitivním Philadelphia chromozomem (Ph+ CML) při selhání nebo intoleranci (intolerance je definována jako nemožnost pokračování v léčbě z důvodu nepřijatelné toxicity) k dasatinibu nebo nilotinibu, podmínkou úhrady ponatinibu v této indikaci je předchozí podání dvou TKI,
1b) u dospělých pacientů v chronické fázi, akcelerované fázi nebo blastické fázi Ph+ CML, kteří jsou nositeli mutace T315I, bez ohledu na linii léčby
2a) u dospělých pacientů s akutní lymfoblastickou leukemií s pozitivním Philadelphia chromozomem (Ph+ ALL), při selhání nebo intoleranci k dasatinibu (intolerance je definována jako nemožnost pokračování v léčbě z důvodu nepřijatelné toxicity) a u nichž současně není z klinického hlediska vhodná následná léčba imatinibem, podmínkou úhrady ponatinibu v této indikaci je předchozí podání dvou TKI,
2b) u dospělých pacientů s Ph+ ALL, kteří jsou nositeli mutace T315I bez ohledu na linii léčby.
Ve všech uvedených indikacích je terapie hrazena do progrese onemocnění, nepřijatelné toxicity nebo transplantace kmenových buněk.
