Dapagliflozin je hrazen jako přídatná terapie u pacientů:
a) s chronickým srdečním selháním s EF menší nebo rovnou 40 % a eGFR větší než 25 ml/min/1,73 m2, u kterých i přes optimální léčbu přetrvává symptomatologie třídy NYHA II až III. Optimální léčbou se rozumí léčba alespoň jedním inhibitorem ACE nebo inhibitorem AT1 receptoru nebo inhibitorem receptoru angiotenzinu a neprylisinu a zároveň beta-blokátorem a antagonistou mineralokortikoidního receptoru. Optimální léčba je podána v cílové dávce dle doporučení nebo v maximální tolerované dávce.
b) se symptomatickým chronickým srdečním selháním třídy NYHA II až III s EF větší než 40 % a eGFR větší než 25 ml/min/1,73 m2 a hodnotou NT-proBNP větší než 300 pg/ml nebo větší než 600 pg/ml v případě pacientů s fibrilací síní.
