Přípravek je hrazen u pacientů se subfoveálně lokalizovanou převážně klasickou chorioidální neovaskularizací objektivizovanou angiografickým vyšetřením. Standardní postup PDT je indikován u klasických a čistě okultních subfoveálních chorioidálních neovaskulárních membrán (CHNVM), u věkem podmíněné makulární degenerace (VPMD), dále u patologické myopie, angioidních pruhů, po traumatech, u syndromu histoplasmózy, u zánětlivých a idiopatických membrán, i při komplikacích makulárních dystrofií (morbus Best, morbus Stargardt). PDT je indikována i tam, kde by další pokles ZO jediného prakticky vidoucího oka pod 0,1 měl nepříznivý dopad na kvalitu života nemocného u komplikovaných případů nemocných se subfoveální CHNVM u VPMD, patologické myopie. Velikost léze je omezena: největší průměr je 5,4 mm. Funkce oka musí být rovna nebo lepší než 0,1.
Podkladem pro indikaci je podrobné biomikroskopické a angiografické vyšetření (fluorescenční, event. indocyaninová angiografie), dále optická koherenční tomografie (OCT).
Indikace k PDT musí být potvrzena vedoucím příslušného pracoviště. PDT bude prováděna na pracovištích, která splní nutné personální i přístrojové podmínky k zajištění dostupnosti pro nemocné.
