Atezolizumab je hrazen v kombinaci s etoposidem a karboplatinou v první linii léčby dospělých pacientů s pokročilým stádiem malobuněčného karcinomu plic (ES-SCLC) splňujících následující podmínky:
a) pacient má výkonnostní stav 0-1 dle ECOG;
b) pacient nevykazuje přítomnost symptomatických mozkových metastáz nebo jsou metastázy adekvátně léčené;
c) pacient není dlouhodobě léčen systémovými kortikosteroidy v dávce prednisonu nad 10 mg denně či jeho ekvivalentu nebo jinou imunosupresivní léčbou;
d) pacient nemá diagnostikované závažné aktivní systémové autoimunitní onemocnění s výjimkou následujících onemocnění: diabetes mellitus I. typu, autoimunitní zánět štítné žlázy, cutaneous autoimunitní onemocnění (např. psoriáza, atopický ekzém, ložisková alopecie, vitiligo);
e) pacient netrpí aktivním intersticiálním onemocněním plic.
Léčba je hrazena do progrese onemocnění, verifikované opakovaným radiologickým vyšetřením v odstupu 4 – 8 týdnů z důvodu odlišného mechanismu účinku imuno-onkologické terapie a/nebo neakceptovatelné toxicity.
